Before becoming a resident at one of our homes a few forms must be completed. Knowing and having the following information available can greatly aid in the process.

• Resident's Full Name
• Known Birth Place
• Social Security Card
• Responsible party's name, address and telephone number
• Emergency party's name, address and telephone number
• Insurance Card
• Medicare Card
• Any attending Physician's Report: with Physician name, address, and telephone number
• Dentist's name, address and telephone number
• Emergency party's name, address and telephone number
• Pharmacy name, address and telephone number
• Advance directives (living will or durable power of attorney for health care
